+ Beneficiary Forms
The IRA change of beneficiary form must be completed when an IRA participant wishes to change his/her designated beneficiary under the plan. When a change of beneficiary form is completed, it supersedes all prior designations of beneficiary and changes the beneficiary on all accounts under the IRA plan.
This booklet must be provided when a participant opens a Traditional IRA, Rollover IRA, or a Simplified Employee Pension (SEP) IRA. When a participant opens a new IRA plan, he/she must complete the IRA adoption agreement and designation of beneficiary form found on pages five and six of the booklet. It is not necessary, however, that a participant opens a new plan (i.e., sign a new adoption agreement and designation of beneficiary form) for each subsequent annual contribution, each transfer-in, or each 60-day rollover from a prior IRA.
This booklet must be provided when a participant opens a Roth IRA. When a participant opens a new Roth IRA plan, he/she must complete the Roth IRA adoption agreement and designation of beneficiary form found on pages five and six of the booklet. It is not necessary, however, that a participant opens a new plan (i.e., sign a new adoption agreement/designation of beneficiary form) for each subsequent annual contribution, each transfer-in, or each 60-day rollover from a prior IRA.
This booklet must be provided when a participant opens a Coverdell Education Savings Account (Coverdell ESA). The adoption agreement and designation of beneficiary form found on pages five and six of the booklet must be completed and signed by the depositor and the responsible individual whenever a Coverdell ESA is opened for a designated beneficiary.
This form must be signed and completed by the trustee of a trust beneficiary before the trust can take a distribution. The form collects important demographic information about the trust and determines whether or not the trust may qualify for an extended distribution period from the account. This is used for IRAs and Qualified Plans where the beneficiary is a trust.
+ Distribution Forms
This form must be completed and signed by the DCP participant and the employer / plan administrator prior to funds being distributed from a Profit Sharing or Money Purchase Pension Plan.
Use this form when a beneficiary of a deceased DCP participant takes a distribution or completes a direct rollover of funds in a Profit Sharing or Money Purchase Pension Plan.
Use this form when the participant wishes to complete one of the following types of transactions: a direct rollover, convert a Traditional IRA to a Roth IRA, reconvert a Roth IRA back to a Traditional IRA, or a recharacterization of a contribution.
Use this form when the participant wishes to complete a trustee-to-trustee transfer from another financial institution.
This form must be completed and signed by the responsible individual or designated beneficiary as appropriate as funds are taken out of a Coverdell ESA.
This form is used in two different circumstances: to transfer an ESA from one institution to another or to transfer the ESA to another designated beneficiary who is a qualified family member.
This form must be signed and completed by the trustee of a trust beneficiary before the trust can take a distribution. The form collects important demographic information about the trust and determines whether or not the trust may qualify for an extended distribution period from the account. This is used for IRAs and Qualified Plans where the beneficiary is a trust.
+ Health Savings Account Forms
This booklet must be provided to each participant when he/she opens an HSA. The HSA adoption agreement found on pages five through seven of the booklet must be completed and signed. This form is also used to designate the beneficiary(ies) of the HSA and to appoint an attorney-in-fact, if the HSA participant chooses to do so.
This form is completed by the HSA participant or the designated beneficiary of a deceased participant when he/she is taking an in-person distribution from the HSA. (This form DOES NOT need to be used if the participant is using a check or debit card to take a distribution.)
This form is used when an HSA participant is moving funds into his/her HSA at your institution from another HSA, an MSA or an IRA.
This form can be given to a HSA participant when he/she makes a contribution to his/her HSA.

Please use this form to request a corrected, duplicate or new tax form.
This form is required to certify the eligibility of rollover contributions. You may also use this form to record and accept contributions from participants. Please note that LT Trust will not record any of the information from this form and will instead collect contribution and rollover information from your institution through our tax reporting procedures.
This form must be signed by the surviving spouse of an IRA participant who wishes to transfer the decedent’s IRA to an IRA for himself or herself. Please note: If the surviving spouse signs an assumption letter, he or she should not sign a cash distribution request to close the deceased spouse’s IRA.
